Case Study: Enabling Independence for an Adult Living with a Progressive Neuromuscular Condition at Home

At Direct Homecare, we provide specialist support for adults living with complex and long-term health conditions, enabling them to remain safe, comfortable, and independent in their own homes. Our care is centred around each individual’s unique needs, while supporting the routines, choices, and lifestyle that are important to them.

This case study focuses on Hailey, an adult living with a lifelong progressive neuromuscular condition and chronic respiratory needs. Through personalised one-to-one live-in support, Hailey is able to continue living in adapted accommodation close to her family, maintaining control over her daily life while receiving the specialist care she requires.

Background

Hailey lives with a lifelong progressive neuromuscular condition, alongside scoliosis, lordosis, impaired swallowing, and chronic respiratory compromise that requires regular non-invasive ventilation. She lives in adapted accommodation adjacent to her family home, allowing her to maintain both independence and close family support. Alongside her dedicated live-in carer, her family remain central to her care, wellbeing, and daily life.

Although Hailey is physically dependent on others for most daily tasks, she has full mental capacity and clearly communicates her wishes. She is actively involved in decisions about her care and values having control over her routines and choices. Supporting her independence means ensuring her preferences are respected while safely managing her complex physical needs.

Hailey leads a fulfilling life and enjoys spending time at work, shopping, attending family gatherings, pursuing hobbies, and being part of her local community. Her care is focused on balancing specialist support with the freedom to continue living life in a way that reflects her interests, routines, and personal goals.

Goals Identified

  • Maintain stable respiratory function and safely manage increasing breathlessness.
  • Support independence within the home while managing complex physical care needs.
  • Maintain safe nutrition and hydration due to swallowing difficulties.
  • Promote comfort, skin integrity, and safe positioning.
  • Enable access to meaningful activities, work, and social outings.
  • Preserve dignity, choice, and autonomy in all aspects of daily care

Interventions & Support Provided

A specialist care package was developed to support the individual with all aspects of daily living while promoting maximum independence. Staff support all transfers using a ceiling track hoist, specialist sling, electric profiling bed, and powered wheelchair, ensuring consent is gained before all interventions.

Respiratory management forms a central part of care. She uses two non-invasive ventilators, including overnight support and additional daytime sessions to manage episodes of breathlessness. Staff are trained to monitor ventilator settings, maintain equipment safety, support oxygen administration where required, and provide reassurance during respiratory distress.

Due to swallowing difficulties, staff prepare all meals in line with soft diet requirements and support safe eating. Meal routines are carefully timed to reduce reflux and fatigue, while hydration and food choices remain directed by the individual herself.

Personal care support includes bathing, dressing, oral care, toileting, skincare, repositioning, and night-time support. Staff adapt routines to her preferences and ensure privacy, dignity, and comfort at all times. Daily skin monitoring is essential due to pressure risks and reduced mobility.

Care also includes supporting access to the community using an adapted vehicle. Staff assist with planning outings, ensuring respiratory equipment accompanies all trips, monitoring fatigue levels, and reducing risks associated with travel. This enables continued participation in work, shopping, social activities, holidays, and hobbies such as gardening.

Outcomes Achieved

  • Consistent respiratory monitoring and proactive intervention has improved safety and reduced distress during episodes of breathlessness.
  • Specialist moving and handling support has enabled safe transfers while maintaining comfort and dignity.
A structured but flexible care approach has enabled the individual to remain involved in family life, and leisure activities.
  • Person-centred support has preserved independence, allowing her to remain in her own home and continue directing all aspects of her care.

Hailey's case highlights how specialist complex care from Direct Homecare can support individuals living with progressive conditions to remain safe, independent, and in control of their daily lives at home. Through a highly personalised and person-centred approach, she has been able to continue living in her own adapted home while managing the increasing complexity of her condition.

By combining specialist clinical support with a strong focus on personal choice, Hailey’s care package promotes both her physical wellbeing and the independence that matters most to her. This enables her to maintain her routines, stay connected to family and community life, and continue enjoying the activities that are important to her.
