This case study focuses on Pedro, an adult male in his late sixties who returned home following a hospital admission with a terminal diagnosis. Upon discharge, he expressed a clear and heartfelt wish to spend his remaining time in the comfort of his own home, surrounded by his family.
Pedro presented with highly complex clinical needs, including a PEG feeding tube, tracheostomy care, oxygen therapy, impaired swallowing, and very limited mobility. He had recently undergone a trial without catheter (TWOC) and was also experiencing deteriorating skin integrity, requiring careful and consistent management. Despite the progression of his condition, Pedro remained cognitively aware and able to communicate his wishes, although this was expected to decline over time.
The primary aim of care was to deliver high-quality, compassionate, and dignified end-of-life support. This included prioritising comfort, respecting Pedro’s preferences, maximising his quality of life, and ensuring his family felt supported and involved throughout his care journey.
We delivered comprehensive end-of-life symptom management, administering anticipatory medications for pain, breathlessness, agitation, nausea, and secretions via PEG and subcutaneous routes in line with strict governance protocols.
A comfort-led PEG plan ensured safe medication administration with appropriate flushing procedures, while nutrition and hydration were provided solely according to the individual’s wishes.
Specialist tracheostomy and oxygen care was maintained through routine cannula management, stoma care, humidification, and minimal-distress suctioning to preserve airway patency and comfort.
A structured skin integrity plan incorporated pressure-relieving equipment, specialist dressings, and comfort-based repositioning to manage a Grade 2 sacral pressure ulcer while respecting personal preferences.
Consistent psychological and family support was provided through calm communication, reassurance, facilitated family involvement, and strict adherence to DNACPR and emergency care planning.
Clear communication, advance care planning, and family collaboration enabled the individual to remain at home his preferred place of care maintaining dignity, emotional wellbeing, and reassurance for loved ones.
