Case Study: Supporting an Individual with Complex Autism and Behavioural Support Needs

At Direct Homecare, we provide specialist support for adults living with complex and long-term health conditions, enabling them to remain safe, comfortable, and independent in their own homes. Our care is centred around each individual’s unique needs, while supporting the routines, choices, and lifestyle that are important to them.

This case study focuses on Gabriel, a young person living with Autism Spectrum Condition (ASC), Attention Deficit Hyperactivity Disorder (ADHD), Sensory Processing Disorder, and Global Developmental Delay. Through personalised 2:1 specialist homecare support, Gabriel is able to remain living safely at home with his family, receiving the structured care and supervision he requires while maintaining his dignity, comfort, and emotional wellbeing.

Background

Gabriel is non-verbal and requires support with communication, emotional regulation, daily routines, and community access. He communicates through a combination of PECS, Makaton, gestures, facial expressions, and behavioural cues, meaning that carers must develop a detailed understanding of his individual communication style to recognise his needs, preferences, and emotions.

Due to his complex needs, Gabriel experiences significant sensory sensitivities, impulsivity, limited awareness of danger, and periods of behavioural distress. He relies heavily on familiar routines, consistent communication, and a predictable environment to help him feel safe and reduce anxiety.

Gabriel lives at home with his family, who play an important role in supporting his care and ensuring continuity. Direct Homecare works closely with Gabriel’s family to maintain consistency in communication approaches, behavioural strategies, daily routines, and activities that are meaningful to him.

His support focuses not only on managing behaviours of concern but also on promoting independence, encouraging engagement, and enabling Gabriel to participate safely in everyday life.

Goals Identified

Gabriel’s personalised care plan was developed to achieve several key objectives:

  • Maintain safety through continuous supervision and proactive risk management.
  • Promote emotional regulation using consistent communication and behavioural support strategies.
  • Maintain structured nutrition and hydration through a strict meal routine.
  • Support personal hygiene, dignity and skin integrity.
  • Enable safe community access and meaningful activities.
  • Encourage communication, choice and engagement while reducing behaviours of concern.

Interventions & Support Provided

A structured 2:1 care package was implemented to provide continuous supervision and reduce risks associated with impulsivity, limited danger awareness, and behavioural escalation.

Staff followed the Behaviour Support Plan, using the Now-Next-Wait approach, positive reinforcement, and calm, consistent communication to promote emotional regulation and reduce distress.

Communication support was provided through PECS, Makaton, gestures, and observation of behavioural cues to ensure needs, choices, and preferences were understood and responded to effectively.

A structured daily routine supported nutrition, hydration, personal care, and independence, with scheduled meals, regular drinks, predictable hygiene routines, and enhanced support during bowel-related care to manage smearing risks.

Environmental safety measures, sensory strategies, and risk-assessed community access were maintained to promote wellbeing while reducing overstimulation and absconding risks.

Outcomes Achieved

Through consistent, person-centred support, Gabriel’s care package has achieved several positive outcomes:

  • Consistent routines and proactive behavioural strategies reduced anxiety, triggers associated with
unpredictability, and episodes of behavioural escalation.
  • Continuous 2:1 supervision improved safety within the home and community, reducing risks associated with
impulsivity, limited danger awareness, and absconding behaviours.
  • Structured communication approaches, including PECS, Makaton, and observation of behavioural cues,
supported emotional regulation, increased engagement, and improved expression of needs and preferences.
  • Planned nutrition, personal care routines, and environmental management promoted wellbeing, dignity,
independence, and reduced distress associated with daily activities.
  • Person-centred planning and risk-assessed community access enabled safe participation in meaningful
activities, supporting quality of life, confidence, and family involvement.

Gabriel’s case highlights how specialist homecare support can enable young people with complex autism, developmental needs, and behavioural challenges to remain safely at home while receiving the structured care they require. Through a highly personalised approach, Gabriel continues to benefit from consistent routines, tailored communication, and compassionate support that prioritises his individual needs.

By combining specialist care with strong partnership working alongside Gabriel’s family, Direct Homecare supports not only his safety and wellbeing but also his ability to participate meaningfully in daily life. This approach enables Gabriel to remain in the environment where he feels most comfortable, surrounded by the people who know him best, while receiving care designed around his unique needs.
